Submit proof of at least 30 days of sobriety. Heart Haven is a private pay program with a weekly fee of $200 and $400 biweekly (no insurance billing).

Once approved, secure your spot and start your journey in a safe, structured recovery environment focused on accountability.

Sober Living at Heart Haven — Housing Application

Thank you for your interest in Sober Living at Heart Haven. This form is used for initial housing eligibility and admissions screening. Heart Haven provides sober living/recovery housing and non-clinical support. We do not provide detoxification, therapy, medical treatment, or clinical treatment. Submission of this application does not guarantee acceptance or housing availability.

Do not submit Social Security numbers, medical records, diagnoses, prescription information, or other sensitive health information through this form.

Admissions Requirements

Heart Haven provides a safe, structured recovery environment. Please review our baseline requirements for residency.

30 Days Sober

Prospective residents must have achieved at least 30 days of continuous sobriety prior to admission to ensure a stable foundation for recovery housing.

Active Engagement

Residents must be actively working, attending school, volunteering, or participating in 12-Step/recovery meetings to maintain structure and accountability.

Fee Structure

Heart Haven is private pay and does not bill insurance. The resident fee is $200 weekly or $400 biweekly.
